    Nutrition Assessment

    In addition to a medical history, an in-depth nutrition assessment should be obtained. Our registered licensed dietitian will work one-on-one with our clients to obtain current and past diet and activity information. This information will then be used to estimate energy intake and nutrient requirements. Many areas addressed are weight, appetite/intake, eating patterns, food preferences/dietary practices, estimation of typical energy and nutrient intake, stress, medication and/or supplement use.

    If you are being referred by your Physician, please include the name, address, phone and/fax. We will work with your physician to provide you with the highest quality care!
